Director of Case Management
Location: Northern California
Our client, a leading acute care hospital is seeking an experienced Director of Case Management to oversee and enhance care coordination, utilization management, and discharge planning. This executive-level position plays a pivotal role in driving patient outcomes, optimizing care transitions, and ensuring regulatory compliance in a hospital-based setting.
Key Responsibilities:
- Provide strategic leadership for the Case Management & Care Coordination department, ensuring high-quality, cost-effective patient care.
- Develop and implement initiatives to reduce length of stay (LOS) and enhance care efficiency.
- Collaborate with hospital leadership, physicians, and post-acute providers to optimize patient flow and transitions.
- Ensure compliance with federal, state, and accreditation standards while maintaining alignment with health system policies.
- Guide and mentor a dedicated team of nurses, case managers, and social workers in delivering best-in-class care coordination services.
Ideal Candidate Profile:
- RN with a California license required.
- Minimum of 3 years in a leadership role overseeing Case Management, Care Coordination, or Utilization Management in a hospital setting.
- Bachelor's degree in Nursing or related field required; Case Management certification preferred.
- Proven ability to drive operational improvements and collaborate across disciplines to improve patient outcomes.
