Clinical Pharmacist (Inpatient & Outpatient)
Midwest - Nebraska
We are partnering with a community-focused hospital in the Midwest that is seeking a Clinical Pharmacist to support both inpatient and outpatient hospital operations. This is a hands-on, patient-centered role with strong clinical involvement and the opportunity to make a meaningful impact within a collaborative care team.
This position is ideal for a pharmacist who enjoys working closely with providers, supporting safe and effective medication use, and contributing to overall patient care outcomes in a hospital setting.
Position Overview:
The Clinical Pharmacist is responsible for supporting medication management across the hospital, including both inpatient and outpatient services (non-retail). This role works closely with physicians, nursing staff, and clinical teams to ensure safe, appropriate, and effective medication therapy.
Key Responsibilities:
- Review, verify, and interpret medication orders for accuracy, appropriateness, and safety
- Provide clinical guidance on medication therapy, including dosing adjustments based on patient-specific factors (age, renal function, etc.)
- Monitor for drug interactions, adverse drug reactions, and medication errors; intervene as appropriate
- Support both inpatient and outpatient hospital pharmacy operations
- Participate in patient education and clinical pharmacy initiatives
- Collaborate with interdisciplinary teams to optimize patient outcomes
- Assist with and support 340B program compliance and optimization
- Contribute to medication safety initiatives, quality improvement, and best practices
Qualifications:
- Graduate of an accredited School of Pharmacy
- Active Pharmacist license (or ability to obtain licensure in Nebraska)
- Experience in hospital-based pharmacy practice preferred
- Strong understanding of clinical pharmacy practices, including medication monitoring and interventions
- Experience or exposure to 340B programs strongly preferred
- Experience with sterile compounding (e.g., IVs, TPN) preferred
